Enrollment is only allowed during specific Open Enrollment Periods. Exceptions are as listed in your brochure and in the policy.
All International Students are required to purchase the plan and will be automatically enrolled in the student health insurance program when tuition is paid. All full-time registered residential students taking 6 or more credit hours who are actively attending classes are automatically enrolled and the premium applied to their tuition bill, for the student health insurance program, unless proof of comparable coverage is provided by the University. All eligible commuter students taking 6 or more credit hours are eligible to enroll in the insurance plan on a voluntary basis through the leonardinsurance.com web site. All eligible commuter students must enroll within the 31 day period starting with the first day of the term of coverage for which premium is paid.
No enrollment form will be accepted beyond 31 days from the effective date for each term of coverage as shown on the enrollment form. The only exceptions are: 1) spouse or dependent child (within 31 days of marriage, birth, adoption.) 2) enrolling as a new or transfer Student (within 31 days of enrollment at the University.) 3) ineligibility under another plan of credible coverage provided COBRA continuation of coverage was accepted and exhausted, if offered.
