You are securely enrolling in your student insurance.

    1.  I understand that I am being charged through my institution for the student insurance premium and that I am eligible for the insurance at my institution.

2.  I want this charge to remain on my tuition bill, and I now wish to set up my account and/or enroll my spouse and/or dependents.

3.  As the insured, it will be my responsibility to make sure the insurance charge is on my tuition bill . Failure to do so, could result in the cancellation of insurance for non-payment.
